My Life Choice Solutions can work with you, plan your carer or your regular doctor to learn about your disability and help to determine the services and supports that you need and that will be part of the application for funding.
No, you don’t require and NDIS plan. You can be self funded or through an agency
Live Choice Solutions can offer you a complete service managing your plan, updates and revisions.
Life Choice Solutions will always endeavour to provide you with a support worker who understands your unique circumstances and is a good ‘fit’ with your lifestyle, however if at any time, this isn’t working for you, you can request another support worker.
Yes you can always add or change services but if you have a NDIS plan and your new service isn’t funded, an application will have to be made to the NDIS for a revision or update to your existing plan. Life Choice Solutions can work with you or your carer on this.
Life Choice Solutions has an office in Toronto and also in Port Stephens with Support Workers located in surrounding areas. If you are leaving the area, Life Choice Solutions will help you to secure supports in your new location.
